Privacy policy

Clinic Intake Web is designed to support clinics that may handle protected health information. Each clinic remains responsible for its own HIPAA policies, workforce training, patient notices, and legally required compliance decisions.

User agreement

Trial account terms for new clinic clients.

New clients must accept this agreement inside the signup form before a trial workspace request is saved. The accepted version, date, IP, and browser information are retained with the request for admin review.

The signup form requires the clinic owner to confirm: I have read and accept the Clinic Intake Web user agreement for this trial account request.

Authorized request

The person submitting the request confirms they are authorized to evaluate Clinic Intake Web for the clinic and to provide accurate account owner, clinic, and contact information.

Trial use only

The trial request is for evaluation. Real patient PHI should not be entered until the production account, subscription terms, and required Business Associate Agreement are active.

Clinic obligations

The clinic remains responsible for HIPAA policies, staff training, user permissions, patient notices, accurate data entry, payer/coding decisions, and compliance with applicable laws.

Vendor limitations

Vendor liability, warranty, uptime, backup, pricing, and service commitments are limited to the final signed subscription terms, BAA, and service agreement.

To the maximum extent allowed by law and the signed service agreement, the vendor is not liable for indirect, incidental, consequential, special, exemplary, punitive, lost-profit, lost-revenue, business interruption, data-entry, payer-denial, or clinical decision damages.

No warranty beyond agreement

Features, uptime statements, backup statements, security safeguards, pricing, trial terms, and deployment timelines are provided for product evaluation and are controlled by the final written subscription, BAA, and service terms.

Customer data and access

Clinics are responsible for accurate data entry, user access decisions, password and MFA device control, staff termination updates, retained copies of exported records, and review of audit and billing activity.
